Pre-School Applicants: Pre-School applicants (who must be 4 years old by September 1st) will need to do the following: [ ] Download and Fill Out the Pre-School Check-Off List Form [ ] Download and Fill Out the Pre-School Application Package [ ] Provide Copy of Birth Certificate [ ] Provide Shot Records (All immunizations must be up to date, including TB. This is required by the State of California Education Code and therefore all requirements must be met before the student will be allowed in school. NO EXCEPTIONS). This is required by the State of California Education Code and therefore all requirements must be met before the student will be allowed in school. NO EXCEPTIONS). [ ] Pay $75.00 Processing Fee ($100 after 1st day of school) - NON REFUNDABLE - for processing application, does not insure enrollment High School Applicants: Grade 9-12 applicants will need to do the following: [ ] Download and Fill Out the Grades 9-12 Check-Off List Form [ ] Download and Fill Out the High School Application Package [ ] Provide Copy of Birth Certificate [ ] Provide Copy of Last Report Card (Grade 9) or High School Transcripts (Grades 10-12) [ ] Provide Copy of SAT 9 Test Results [ ] Provide Sports Physical Report (if participating in athletics - including Cheerleading) [ ] Provide Shot Records (All immunizations must be up to date. This is required by the State of California Education Code and therefore all requirements must be met before the student will be allowed in school. NO EXCEPTIONS).
